Services are offered Monday-Friday, 8:00 am-5:00 pm. All county residents are eligible for services but priority is for high risk, medically fragile maternal and child clients. Infants 0-36 months of age who are at risk of becoming developmentally delayed, abused, neglected or of failure to thrive because of biological, environmental and/or psychosocial factors will be accepted for PHN services. These services include assessment, education, emotional support, referral, case management and interagency coordination.
